Medicare Demonstration

The Medicare Demonstration, (locally known as Connect Care), was implemented in West Virginia and Ohio by CAH in collaboration with Monroe County Long Term Care Program, Inc. in Rochester, NY.  From 1997 - 2002 the demonstration tested a Medicare Waiver benefit for:

Consumer-Directed In-Home Services:
Participants could receive up to $250 per month, (with a 20% co-pay), to purchase personal care, respite, transportation, home modifications, consumable care goods, and medical supplies and equipment. 

Health Promotion Nurse Services:
A nurse worked closely with primary care physicians to provide participants and their family caregivers with consultation about medications, self-care, disease management, lifestyle change, and patient-provider communication skills.
